What to expect on your first visit:
Our goal at Highland Foot and Ankle Clinic is to make your appointment as complete and thorough as possible.
We encourage you to fill out the forms before your first appointment. Forms can be mailed to you at your request if you do not have access to a printer, otherwise please arrive fifteen minutes before your scheduled appointment time to fill these forms out at the clinic. Please bring the following items:
- Insurance card(s)
- Insurance co-payment
- Photo identification such as a drivers license
- List of current medications & allergies
- List of previous surgeries and dates they were performed
Providing us with your complete medical history will help our physicians recommend the most appropriate medical treatment for you as an individual. If you are seeing us for a second opinion, bringing previous medical records and/or radiology reports would be helpful but not necessary. While we accept most insurance plans, coverage depends on your individual policy. Medical billing is handled by our staff. While our staff may be able help with some questions, we strongly encourage you to contact your insurance company for your specific coverage questions.
